About Our Downeast Primary Care Practice
It’s time for a new breed of primary care.
Black Dog Medical is a membership-based primary care practice that aspires to be your companion in good health. After 20+ years caring for patients in Ellsworth, Maine, Dr. Patty Wyshak designed her own practice so she could be there for patients when you need her — whether you’re a young adult or retired and ready to thrive.
Here are just a few ways Dr. Wyshak helps keep our patients healthy and happy:
Get answers and care when you need it…not weeks later.
Dr. Wyshak and the Black Dog Medical team will respond to your needs and concerns as soon as possible, usually within hours or sooner, for urgent matters.
Visits will take as long as needed, and we’ll do our best to see you at your appointment time. When we’re running late, we’ll give you a heads up. In other words, we’ll treat you the way we love to be treated.

We use the same modern, secure electronic health records (EHR) software that top physician practices and urgent care centers are using.
Dr. Wyshak can care for Maine residents by phone, messaging or even video chats.
Stay well while traveling
Thanks to technology like video calling, you can get medical advice and even have prescriptions filled when you’re out of town or home sick.

You choose where to get your prescriptions.
We accept Medicare and insurance.
We have set up relationships with insurers and Medicare for covered services such as wellness exams, evaluations and prescriptions. Currently, Black Dog Medical and Dr. Wyshak are in-network for the following insurance providers:

- Anthem Blue Cross Blue Shield
- Aetna
- Harvard Pilgrim
- Geisinger Health Plan
- Community Health Options
- Feel free to ask us if you’re have insurance other than these.
There are no co-pay or out-of-pocket costs for office visits beyond the membership fee.
NOTE: Black Dog Medical is not an insurance plan. We recommend that patients also have insurance as a safeguard.
Need a second opinion? No problem.
Your membership also includes Second Opinion consultations by Dr. Wyshak, when you’re under the care of specialists. We’ll help you make the right decisions for you and get the care you need.
Membership Fee
Black Dog Medical’s membership fee is similar to a club membership fee, with the big benefit being access to a top-dog doctor.
Credit card payments have a 3% surcharge to cover our processing costs.
